We are currently recruiting for a Recovery Worker T4 Coordinator.
Role / Team overview
Working within an integrated healthcare framework to support drug and alcohol users aged 18 and over (dependent on service). Services deliver Psychosocial Drug and Alcohol Interventions for Service users with alcohol or drug problems. Services provide client-centred treatment using harm reduction and abstinence approaches, tailored to individual needs.
Responsibilities include:
- Managing caseloads, providing harm reduction advice, assessments, care planning, and delivering 1:1 and group sessions, including structured treatment programmes.
- Collaboration with services such as Housing, Employment, Healthcare, Probation, and CRCs is essential.
- Contributing to targets set by Forward and local commissioners, with a focus on measuring outcomes through cooperative agency relationships.
- Overseeing service users requiring inpatient treatment, supporting them through funding applications, attending panels, exploring residential options, and ensuring pre-treatment preparation.
- Acting as the main contact for residential facilities, maintaining up-to-date information.
Flexibility is required, including some evenings/weekends with reasonable notice, to ensure service continuity.
Key responsibilities:
- Conduct comprehensive assessments to inform treatment plans, including risk assessments.
- Deliver interventions tailored to individual needs, including 1:1 and group work.
- Partner with other agencies, including training, employment, and family support services, to meet client needs.
- Coordinate with healthcare providers to monitor and manage client health and risks.
- Develop and contribute to assessment and care plans, supporting engagement with specialist services.
- Motivate clients to engage with support services as outlined in their care plans.
- Assist clients with their care plans, including referrals to community support, housing, benefits, and employment services.
The ideal candidate will have:
- A Level 3 Diploma in Health and Social Care and/or qualifications in substance misuse.
- Experience working within substance misuse services.
- Experience conducting comprehensive assessments, risk management, and creating SMART care plans.
- Experience delivering structured interventions and facilitating groups.
- Knowledge of motivational interviewing techniques in individual and group settings.
