Key Responsibilities- Deliver structured Atrial Fibrillation (AF) reviews in line with clinical guidance.
- Review and optimise anticoagulation therapy, including DOACs.
- Support the achievement of QOF and Enhanced Care Framework (ECF) requirements.
- Identify patients requiring intervention to improve safety and outcomes.
- Provide medicines optimisation support and clinical advice to the wider team.
- Maintain accurate clinical records and coding.
- Communicate effectively with patients and healthcare professionals.
- MPharm degree or equivalent.
- Current GPhC registration.
- Strong understanding of QOF and ECF frameworks.
- Experience managing anticoagulation, including DOACs.
- Previous experience within a GP practice or PCN.
- Excellent communication and consultation skills.
- Independent Prescriber qualification.
- Experience using EMIS or SystmOne.
