About The Role
We are recruiting a compassionate and motivated Community Link Worker to support individuals referred via GP practices as part of a Social Prescribing service within Derby City South Primary Care Network. This role focuses on supporting people who may be experiencing social, emotional or practical challenges that impact their health and wellbeing, such as loneliness, mental health difficulties, housing issues, financial hardship or long-term health conditions. Using a person-centred and trauma-informed approach, you will work alongside individuals to understand what matters to them, co-produce personalised wellbeing plans and connect them to appropriate community, voluntary and statutory services. The aim is to empower individuals to improve their independence, resilience and overall quality of life.
Key Responsibilities
- Receive and manage referrals from GP practices
- Build trusting, person-centred relationships with individuals
- Take a holistic approach to understanding needs, strengths and goals
- Co-produce personalised wellbeing action plans
- Support individuals to access community groups, services and activities
- Encourage independence, self-management and social connection
- Use coaching and motivational approaches to support behaviour change
- Identify and manage risk appropriately, escalating concerns where required
- Maintain accurate records using clinical and organisational systems
- Work collaboratively with GP practices, NHS partners and community organisations
Partnership and Community Engagement
- Build effective working relationships with GP practices and PCN staff
- Maintain up-to-date knowledge of local community services in Derby
- Promote social prescribing and its benefits across health and community settings
- Support the development of strong referral pathways between services
What We’re Looking For
Essential
- Experience supporting individuals in a health, care, community or voluntary setting
- Strong person-centred and strengths-based approach
- Ability to build rapport and work in a non-judgemental way
- Experience managing or coordinating support for individuals
- Good understanding of social issues affecting health and wellbeing
- Confident working independently and managing a caseload
- Strong communication and IT skills
- Full UK driving licence and access to a vehicle
- Enhanced DBS (or willingness to obtain)
Desirable
- Experience within the voluntary and community sector
- Knowledge of local services in Derby
- Training in motivational interviewing or coaching
