We’re looking for a passionate and proactive Health and Wellbeing Practitioner to join our team at HMP Cardiff on a full-time basis (37.5 hours per week). This is an opportunity to support individuals on their journey towards positive change, helping them improve their health, wellbeing, and prospects for a successful return to the community.

Salary: GBP 26,787 per annum

What You’ll Be Doing:
- Managing a caseload of service users and coordinating their treatment journeys
- Conducting assessments and developing person‑centred care plans
- Delivering structured 1:1 interventions and facilitating group sessions
- Providing harm reduction advice and wellbeing support
- Supporting release planning and through‑care arrangements for individuals returning to the community
- Carrying out second signatory duties and supporting clinical services
- Working closely with multidisciplinary teams and partner agencies
- Maintaining accurate case records and performance data to meet service targets

All prison‑based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete.
