We are looking for a passionate and motivated Health and Wellbeing Practitioner to join Forward Trust's Connections Team at HMP Millsike. This full‐time position focuses on supporting people in custody to improve their physical, mental, and emotional wellbeing, build resilience, and develop healthier lifestyles that can be sustained on release.

Responsibilities

  • Deliver one-to-one support, group interventions and wellbeing workshops.
  • Coach and mentor service users to achieve positive, lasting change.
  • Support and develop prisoner Health and Wellbeing Champions.
  • Promote healthy lifestyles, recovery, and wellbeing across the prison.
  • Organise health promotion events and awareness campaigns.
  • Build strong partnerships with healthcare, mental health, and substance misuse services.
  • Support the delivery of a varied programme of wellbeing activities, including evenings and weekends.
  • Maintain accurate records, care plans, and case‐management documentation.

Qualifications & Requirements

  • Passionate about health, wellbeing and recovery.
  • Skilled at building positive relationships and motivating others.
  • Confident delivering both one-to-one and group-based interventions.
  • Organised, proactive and able to manage a varied caseload.
  • Comfortable working within a multidisciplinary environment.
  • Committed to safeguarding, inclusion and person‐centred support.

Additional Requirements

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks. Please note: if you have lived overseas within the last 5 years then checks may take longer.
