Overview

Looking for a role where you can genuinely change lives? We are looking for a passionate Health and Wellbeing Practitioner to join our team at HMP The Mount, supporting individuals on their journey towards recovery, better mental health, and a positive future beyond custody. This is a full-time position (35 hours per week).

Responsibilities

  • Managing a caseload and delivering personalised support and care plans
  • Completing assessments, risk reviews and structured interventions
  • Delivering 1:1 sessions and group programmes on substance misuse and low-level mental health topics
  • Providing harm reduction advice and health and wellbeing guidance
  • Supporting service users with release planning and continuity of care into the community
  • Working collaboratively with healthcare, probation, housing, employment and external recovery services
  • Maintaining accurate case records, reports and outcome data
  • Contributing to service performance targets and quality standards

Qualifications

  • Experience (or a strong interest) in substance misuse, mental health or wellbeing services
  • Strong communication and report writing skills
  • Good organisational skills and attention to detail
  • Confidence using IT and case management systems
  • A proactive and solution-focused approach
  • Understanding of safeguarding, professional boundaries and confidentiality
