Long Term Health Conditions Buddy
Hull CVS is looking for volunteers for our Long‑term Health Conditions Buddy Service, to help others through their care journey. If you have experience of living with a long‑term health condition or have cared or supported others with the same, we would like to hear from you.
The role will involve offering a friendly ear to provide practical support and advice over the telephone to those who are living with a long‑term health condition. Days and times are flexible and can work around you, and there are also options to volunteer from home.
Tasks and responsibilities:
- Calling on the phone and checking on the clients' wellbeing.
- Offering your lived experience to provide peer support to others.
- Escalating any healthcare/social care needs when needed.
- Clear and accurate record keeping and submitting of case notes to project lead.
- Signposting clients to appropriate and helpful information, services and support groups.
