We are seeking an experienced Care Navigator to provide support across GP Practices and Primary Care Hubs in Sefton and the wider Liverpool area. As a Care Navigator with PRIMARY CARE experience, you will be a vital link between patients, doctors, and other healthcare professionals.

We are looking for a skilled and dedicated Care Navigator to join our team on a contract basis. The Care Navigator will provide administrative support to GP Practices and Primary Care Hubs, ensuring that patients receive the best possible care. If you have previous experience as a Receptionist or Care Navigator within a GP or health centre, we would love to hear from you.

Responsibilities
  • Provide administrative support to GP Practices and Primary Care Hubs
  • Handle patient queries and provide information on services and procedures
  • Work closely with healthcare professionals to ensure seamless patient care
  • Maintain accurate records and databases using EMIS
  • Offer patient navigation duties and provide support to patients
Qualifications
  • Experience as a Care Navigator or Receptionist within a GP or health centre is preferred
  • Strong communication and organizational skills
  • Familiarity with GP administrative processes and EMIS or similar systems
