We are working with a growing specialist adult care provider to appoint a Referrals & Assessments Manager into a pivotal commercial and operational role. This position sits at the heart of the organisation’s growth strategy and plays a critical role in ensuring people are placed into the right services, at the right time, with a strong focus on quality, outcomes and occupancy.
You will lead referral activity across multiple local authority areas, working closely with commissioners, operational leaders and service managers to deliver effective placements and sustainable growth.
Key Responsibilities- Own and manage the full referral lifecycle from enquiry to placement
- Serve as the key contact for social workers and commissioning managers
- Conduct and lead referral assessments for new placements
- Negotiate placements and contractual terms with local authorities
- Build and maintain strong commissioner relationships across the Midlands
- Work collaboratively with operational teams to maximise occupancy
- Track referral performance, conversion rates and trends using CRM systems
- Significant experience in a Referrals / Placements / Transitions role within adult social care
- Strong understanding of commissioning frameworks and local authority processes
- Confident in assessing referrals for suitability and risk
- Experience within autism, learning disability or complex needs services
- Commercially astute, relationship-driven and operationally grounded
- Influential role with visibility across operations and commissioning

Due to the nature of the role and organisation, the successful candidate will need to be able to travel on a regular basis across the midlands region. Additionally, due to the location of the services, you will need to hold a valid driving licence and have access to your own vehicle.
