Job Title: Health & Care Improvement Officer

Salary: £45,624 – £52,038 per annum

Grade: F

Contract: Fixed Term Contract (2 years)

Full-Time: 37 Hours per Week

The role is responsible for the development and implementation of the Adult Social Care transformation programme – as part of Better Care Together Thurrock and the Integrated Care Strategy.

The role sits within Adult Social Care – but working in partnership with Housing and local NHS and community partners.

Our communities have been front and centre of our transformation journey to date, and the role will support the development and implementation of our partnership with the strong, diverse and vibrant communities that we serve.

Experience and Skills

  • A good understanding of health and social care.
  • A good understanding of communities and voluntary sector agencies.
  • Experience of delivering change initiatives.
  • The ability of working in partnership with others.
  • A Standard Disclosure and Barring Service (DBS) check is required.
