To work as a member of the Families First Pilot Programme (FFPP) to promote children living with their families where appropriate and safe to do so, taking a lead role in a specific area e.g. Domestic Abuse, Substance Misuse.

To be the case holding ‘Lead Professional’ working with families where domestic abuse or substance misuse is a concern. This is an exciting opportunity to develop practice in these areas within the Families First Pilot.

To support the development, delivery and co-ordination of an agreed plan as part of a ‘Team Around the Family’, within the context of the FFPP Pilot programme. To promote the use of community-based services to enable families to find local solutions and build resilience to sustain agreed outcomes.

Key service accountabilities:
  • To be the case holding ‘Lead Professional’ on a wide range of cases that correspond with the post holder’s skill, experience, expertise and the needs of the family.
  • The Family Support Worker will be part of a ‘Team Around the Family’ and will take the lead in co-producing a plan with the family and professionals that will help the family to find local solutions and build resilience to sustain agreed outcomes.
  • To organise regular meetings with the family and professionals in order to review the plan and ensure it has clear outcomes.
  • To demonstrate and model a high standard of practice that reflects a whole family, strengths-based approach when working with children and families.
  • To work in partnership with colleagues and other professionals from a range of organisations to develop a service that delivers the best possible outcomes for vulnerable children, young people and families.
  • To undertake a whole family assessment of need which will include liaison, as appropriate with other professionals and key people, e.g. Health Visitors, Schools, Housing and Education Departments.
  • To maintain up to date and effective case records in line with the performance targets set within the Prevention and Support Service.
  • To provide practical assistance, support and guidance to parents to enable them to care and protect their children successfully.
  • To support families to establish good local community networks and support mechanisms to promote their self-help skills and resilience.
  • To encourage parents to keep medical and other appointments linked to their family’s development, which may include transporting them to appointments and meetings.
  • To advise and support parents to establish routine in providing appropriate levels of care for their children.
  • To undertake direct work with families and children of all ages.
  • To support and advise service users and to advocate on their behalf if necessary.
  • To ensure there is support within the home on budgeting, financial management, diet, home conditions and the provision of nutritious meals.
  • At the discretion of senior staff, any other activities that may be reasonably required from time to time commensurate with the nature of the post.
  • To take the lead in delivering training or support the delivery of training to professionals and to parents.

Special Conditions: Due to the nature of the post ‘out of hours’ (evening) and weekend / bank holiday work will be necessary for which time off in lieu will be provided or appropriate payments for out of Band hours and Bank Holiday working.
