It’s a really exciting time to join us as we begin to deliver the Heathier You NHS Diabetes Prevention Programme (NDPP) throughout the UK and we’re looking for a team of remote Triage and Support Officers to join us!
The NDPP programme is designed to support individuals to make long term behavioural change in order to help halt or reverse the progression of Type 2 Diabetes over a nine-month period. As Triage and Support Officer, you will be the first point of contact for our clients, providing excellent customer service and helping individuals get enrolled on the programme.

A typical day as a Triage and Support Officer might include:
- Delivering exceptional customer service whilst managing client referrals over the phone and electronically.
- Inputting all client referral details into our database accurately.
- Motivating service users to achieve their health outcomes, ensuring that all screening is carried out to the highest standards in accordance with NHS Code of Practice
- Working collaboratively with the rest of our National Diabetes Prevention Programme (NDPP) team as well as our partner organisations to create referral routes and help meet targets
- Prioritising key tasks depending on the needs of the service.
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

Contract:Zero hours contract, bank/sessional staff.
Salary:£12.50 per hour base rate plus additional performance related bonuses.
