We’re looking for a Triage and Support Officer to join our Thriving Nottingham service. Thriving Nottingham is an Integrated Wellbeing Service which has been supporting local residents since April 2024, with Smoking Cessation, Weight Management, Physical Activity, Child and Family programmes, and Health Coaching.

A typical day as a Triage and Support Officer might include:
- Using a variety of systems and approaches to ensure clients get the best, positive, and client-centred approach when supporting them through our onboarding process.
- Offering guidance and support to clients both on the phone and in written format to ensure they are best aligned to the most appropriate health intervention offer for them and their needs. (Training will be provided if successful).
- Contacting clients daily who wish to start their journey with us, being a friendly, warm, and empathic voice that represents our service positively and correctly recording their information on our bespoke systems accurately and timely.
- Responding to requests for information from clients, partner organisations, and the public.
- Taking responsibility for your own training and development, that way we can make the biggest impact!
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements, and guidance.
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

Contract: permanent, full time 37.5 hrs (We have 2 vacancies)
Salary: Starting at £22,500

Location: This job is based in Nottingham. You will be expected to work an evening per week and Saturdays on a rota basis.
