Thrive Tribe is seeking a Health and Wellbeing Coach to join the Primary Care Community Wellbeing team across the East Coast Lincolnshire network.
You will deliver health coaching to support vulnerable patients, travel between GP practices and community venues, and work with GPs, nurses and link workers to provide a holistic care approach.
