Thrive Tribe is recruiting a Navigator to support onboarding for residents across City & Hackney. You will motivate clients toward health outcomes, ensure high-standard screenings, and guide them to appropriate health pathways.
You will also log referrals in the case management system, respond to information requests, and work with colleagues to develop services while upholding information governance and Thrive Tribe values.
