It’s a really exciting time to join us as we begin to deliver the Healthier You NHS Diabetes Prevention Programme (NDPP) throughout the UK and we’re looking for a team of remote Triage and Support Officers to join us!
The NDPP programme is designed to support individuals to make long term behavioural change in order to help halt or reverse the progression of Type 2 Diabetes over a nine‑month period. As a Triage and Support Officer you will be the first point of contact for our clients, creating an excellent and safe experience for them.

Responsibilities- Deliver exceptional customer service whilst managing client referrals over the phone and electronically.
- Support service users to start the National Diabetes Prevention Programme.
- Input client referral details into the database accurately.
- Motivate clients to achieve their health outcomes, ensuring that all screening is carried out to the highest standards in accordance with NHS Code of Practice.
- Work collaboratively with the NDPP team and partner organisations to create referral routes and help meet targets.
- Identify and maintain your own training needs.
- Work with colleagues across all Thrive Tribe services to contribute to new projects and service developments.
- Ensure due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
- Help us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and embed our values.
