We’re looking for a Stop Smoking Partnership Delivery Lead to join our One You Lincolnshire service. One You Lincolnshire is an Integrated Healthy Lifestyle Service which has been supporting local residents, since July 2019, with Smoking Cessation, Weight Management, Alcohol Reduction, Physical Activity and Health Coaching. More recently the service has expanded to offer Child and Family Weight management, and Strength and Balance for adults aged 65+.

Responsibilities- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!
- Providing line management to a team of programme coordinators and delivery staff, working together to innovate and support both delivery partners and clients.
- Work closely with the Head of Smoking Cessation to support partner organisations and colleagues in Primary and Secondary care to maintain client uptake and meet targets.
- Work closely with other Stop Smoking Team Leads to reach targets, reflect and plan for areas that require improvement and be a driven lifelong learner in the field of Stop Smoking.
- Develop and support delivery of Tier 1.5 and Tier 2 Stop Smoking pathways.
- Develop and support delivery of Relapse Prevention programme.
- Develop and deliver updates and reports for wider team.
- Support with ordering and stock control of NRT, remote CO monitors and consumables to meet contracted requirements.
- Monitor outcomes and performance to deliver on contracted KPIs.
- Ensure an excellent and safe experience for residents using our policies and procedures.
- Work with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
- Ensuring due consideration is given to our Safeguarding, Information Governance and Clinical Governance policies and procedures, including DBS checks.

To work with us in this role it is desirable to have relevant leadership experience and project management. A relevant qualification (such as NCSCT Stop Smoking Advisor training) is a bonus! Salary: Starting at £28,000 per year

Location: This job is a hybrid remote role based in Lincolnshire, with regular travel across the County (car owner required). We require the right to work in the UK.
