Main Responsibilities of Rehabilitation Assistant:
- To assist the people we work with to become more independent with their physical care, personal hygiene, daily life tasks and skills with a person centred approach.
- To participate in the creation of a clinical led rehabilitation plan and its implementation, ensuring that their independence is preserved and developed as far as possible and affording appropriate levels of privacy and dignity.
- To maximise the use of the environment and tools within the service to promote independence.
- To participate in a multi disciplinary approach by offering rehabilitation focused feedback to aid clinical planning and reviews.
- To be active participants in rehabilitation goal setting.
- To be confident and competent in explaining their rehabilitation role to the people we work with, family and other professionals.
- To promote effective customer communication with their relatives, friends, professionals and other members of staff, in order to ensure the continuity of quality and safety in the provision of support.
- To work within a person-centred approach to encourage the people we work with to maintain maximum independence, considering their physical, sensory, cognitive and emotional condition, as well as their current skills, personal choices and aspirations.
- To encourage the people we work with to participate in the running of the service provision.
- To be open to peoples preferences as to how tasks are carried out.
- To report and record any incidents or events that may have an impact on the persons PCP (Person Centred Plan), liaising with the immediate supervisor (and/or any other relevant staff) as appropriate.
- To ensure that record-keeping is maintained and updated in an accurate, timely and confidential manner.
- To become familiar with any changes in the peoples condition or situation at the beginning of each shift, as well as any associated changes in the working environment or premises.
- Where required, to escort and assist people with a disability both in and around the service setting and accompany them to other external events and appointments in the wider community.
- To support with daily life skills such as shopping, food preparation and general housekeeping in accordance with the requirements of the service.
- To actively encourage and participate in any programmes (e.g. exercise) identified in a customers PCP by the relevant therapist.
- To help customers with mobility and other physical or sensory disabilities and assist in the use of support aids and personal equipment.
- Where required, administer medication and/or delegated nursing care as directed by nurses, medical advisors and supervisors or line-manager.
- To comply with all health and safety policies and procedures.
- To comply with Threshold and any local safeguarding and Mental Capacity Act (MCA) policies and procedures.
- To work within the regulatory compliance requirements applicable at the respective service.
- To maintain an awareness of individual customer risk assessments relating to specific support needs, for example, dysphagia, mobility and safety.
- To participate in supervision, staff meetings and training activities (including updates) as required, sometimes off-site and outside of normal hours.
- To be responsible for maintaining and improving own knowledge and skills through experience and training.
- To undertake any other additional responsibilities as requested by the Service Manager following the successful completion of any specific training and personal skills development.
- To undertake any other reasonable duties as requested.

- Act as a Deputy Fire Warden and / or First Aid Person if designated.

Person Specification:

- Essential: GCSE English and Mathematics Grades A C or demonstrable equivalent & Lvl 3 RQF Qualification or equivalent & Minimum 1 year experience working in following: -Working in complex needs setting -Be familiar with the needs assessment and rehabilitation planning process & To hold a level 2 in food hygiene or be willing to attain it once in post. Or a psychology degree (lv 6) or other relevant discipline or equivalent & To hold a level 2 in food hygiene or be willing to attain it once in post & Lvl 3 RQF Qualification or willing to attain in post through organisation.
