We're hiring a RNLD Clinical Lead at Thornbury Community Services (part of Acacium Group). This is a permanent, full-time position field-based across North Yorkshire with a salary of £49,000-£53,000 plus a £3,000 car allowance.
We are looking for a RNLD Clinical Lead to join Thornbury Community Services, an 'outstanding' CQC-rated care provider that delivers complex care at home or in the community. In this role, you will manage a caseload of packages to support individuals with learning disabilities, autism, and mental health needs.
In this role, you will:
- Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family, and the care team.
- Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained.
- Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
- Support the care team by delivering client-specific clinical training, mentorship, and supervision by providing feedback, conducting performance reviews, and supporting professional development.

- Experience leading community teams to support individuals with learning disabilities, autism, and mental health needs.
- A valid driver's license and access to a vehicle.
