Thistle Foundation is seeking a part-time Health & Wellbeing Practitioner to join our Wellbeing teams across Midlothian GP Practices.

You will manage a caseload, deliver self-management coaching, and run group Lifestyle Management courses to support people with long-term health conditions.

The role requires travel between GP practices and community venues, working autonomously in the community, and contributing to the Macmillan, Improving the Cancer Journey contracts.
