We are looking for a full time Practitioner to join Thistle’s Wellbeing teams located in Midlothian GP Practices. Are you a health and social care practitioner with experience of supporting people living with Long Term physical and mental Health Conditions to self-manage? Do you cope well supporting vulnerable people who experience significant psychosocial barriers and health inequalities? Do you believe people have strengths, are resilient and are experts in their own lives? Do you thrive working autonomously in the community setting? Do you have good solution focused coaching and group facilitation skills? If you do, then we would like to hear from you!

The post holders will:

  • Work with a team of practitioners to deliver our Midlothian GP contract
  • Be prepared to travel across Midlothian to engage with people referred to the Wellbeing Service.
  • Practice persistent engagement to help people who struggle to receive support
  • Manage a caseload of people referred to the service
  • Use a strengths based personal outcomes approach to help people identify and access appropriate support for them to self-manage including:
    • Providing specific coaching in self-management
    • Delivering group based Lifestyle Management courses
    • Linking people to community resources and supports
  • Support volunteers who have lived experience of long-term conditions into peer roles

Location: Although predominantly based in one or two GP practices, travel to other GP practices is required to engage with people referred to practices that may not have a regular practitioner presence. Also required to travel to a variety of community venues to deliver courses across Midlothian.
