Role Overview: Thirteen Group is on the lookout for a Support Service Co-Ordinator to join our Specialist Housing team at our Supported Housing Scheme in Hendon, Sunderland. You will be responsible for delivering high-quality, holistic, and person-centred housing support services to a diverse range of Customers in line with legal, contractual, and regulatory requirements to facilitate positive move-ons - preparing customers for independent living by embedding tools to develop essential skills through effective support planning.
