Thirteen Group is looking for
Living Well Co-ordinator tojoin our Specialist Housing team. We have a permanent part time role available.
Our extra care, sheltered schemes and sheltered bungalows in the community offer housing for people aged 55 plus who need housing related support to live independently in their own home.

This is a roam role, and you’ll be based within one of sheltered schemes, but you’ll be expected to cover other schemes across the Tees Valley when required.
We’ll need you to be flexible around business and team need, and you’ll also need to travel to our fantastic office at Hudson Quay in Middlesbrough when required.

Key Responsibilities
- Provide personalised, high-quality housing management and support services to help customers feel safe and be supported to live well and independently for as long as possible.
- Implement individual support plans to identify risk factors, safeguarding concerns, and other interventions.
You’ll build professional relationships to ensure customers feel secure and safe in their residential environment.
- Work in partnership with families, care providers, health professionals, and other organisations to promote healthy lifestyles amongst customers.
- Deliver community engagement and wellbeing initiatives.
You’ll organise inclusive social events, in collaboration with charitable organisations to create a settled and vibrant community.
- Manage schemes, ensuring buildings are managed in line with health and safety requirements, including the completion of compliance checks.
- Support customers with benefits advice and financial inclusion opportunities across different accommodation types.
About You
- We’re looking for someone who is a skilled influencer and communicator, and you’ll be passionate about providing a high-quality service.
- You’ll have proven experience of working with or supporting older and/or vulnerable people, with an understanding of support plans and risk assessments.
- In-depth knowledge of safeguarding policies and procedures, the benefits system and financial inclusion. You’ll also have a track record of handling housing management issues.
- With empathy at the heart of your approach, you’ll be great at communicating complex information effectively to diverse audiences, with the ability to solve day to day problems and deliver a customer-driven service,
You’ll have a full UK driving licence and access to a vehicle.
