Thirteen Group is seeking a Living Well Co-ordinator to join our Specialist Housing team on a permanent part-time basis.

The role is based in sheltered schemes across the Tees Valley, with travel to other schemes and the Middlesbrough office when required.

You will provide housing management, support plans, and collaborate with families, care providers, and health professionals to promote wellbeing among customers aged 55 plus.

DBS check is required.
