Thirteen Group invites applications for a Support Service Co-Ordinator to join our Specialist Housing team at the Supported Housing Scheme in Hendon, Sunderland. The role focuses on delivering high-quality, holistic, and person-centred housing support services to a diverse range of customers in line with legal, contractual, and regulatory requirements, facilitating positive move-ons and preparing customers for independent living through effective support planning.

Responsibilities

  • Deliver practical tasks and flexible support solutions to meet diverse client requirements and promote independence.
  • Provide flexible, person-centred support using PSI interventions and practical assistance; develop, implement, authorise, and review support plans in response to customer needs and the service.
  • Meet performance objectives and service criteria while adhering to legislation, safety protocols, and contract requirements.
  • Plan, coordinate, and deliver effective and engaging resident meetings and activities.
  • Act as a key representative in safeguarding and multi-agency meetings, delegating attendance where appropriate.
  • Report and monitor safeguarding concerns, coordinate and deliver appropriate actions, and produce accurate and timely information.

Qualifications

  • Possess an NVQ Level 3 qualification or equivalent demonstrating competency in support service coordination.
  • Know the principles of effective support planning and delivery.
  • Have a thorough understanding of safeguarding, equality and diversity, Health & Safety, lone working and risk-management policies.
  • Be proficient in utilising information technology and media platforms to deliver effective service outcomes.
  • Have emotional resilience and confidence to handle complex situations whilst working independently.
  • Demonstrate expertise in supporting customers with varied needs to achieve independence and lead fulfilling lives.
