We are looking for a Support Service Co‑ordinator based at our young parent supported housing scheme at School View in Durham to support young families that need assistance to live more independently.

Key Responsibilities
- Run activities linked to seasonal events or celebrations or activities that help improve social isolation and specific learning linked to support.
- Support with any referrals received and assess suitability for the service.
- Work with individuals to create person‑centred support plans and hold weekly keywork sessions to achieve their goals identified.
- Manage risk and safeguarding for all residents.
- Attend multi‑agency meetings and provide written reports.
- Manage void properties and allocations.
- Responsible for managing building compliance and health & safety of the scheme.
- Assist individuals to maximise their income and apply for relevant welfare benefits and grants.
- Work as part of a team of four across a two‑week rota, covering operating hours of 9 am to 5 pm on weekdays.
- You’ll be required to work one in two Saturdays and will have the Monday after off.
- You’ll also be expected to be on call one in every four weeks.
- Have the opportunity to pick up additional hours on a relief basis, working across other schemes to gain insight into other parts of the business and create strong working relationships across teams.
Qualifications
- Experience supporting vulnerable client groups, with a good understanding of adult and child safeguarding and the complexities that it might bring.
- Ability to thrive in lone working at times, as well as part of a team.
- Essential to have a driving licence and access to your own vehicle, as the role involves supporting complex families on an outreach basis across County Durham.
- Able to prioritise workload and supportive in a non‑judgmental way, responsible for enabling customers to reach their potential and identifying their goals through individual support plans.
- Emotional resilience and the ability to deal constructively with challenging behaviour.
