Join us as a Family Intervention Co-ordinator at Thirteen Group!

Thirteen Group is on the lookout for two Family Intervention Co-ordinators. You will coordinate and deliver personalised support to families with complex needs, helping them to overcome barriers and achieve lasting, positive change. If you have a passion for supporting families to thrive, this is the role for you!

This is a roam role, so you will mainly work within communities across the Tees Valley, as well as home and at least one day a week from our fantastic office at Hudson Quay in Middlesbrough. We will need you to be flexible around business and team needs, so you may need to come into the office more frequently when required.

Key Responsibilities

  • As a key worker for families with complex needs, you will coordinate and deliver personalised support plans and proactively track outcomes.
  • Build trust and positively engage with families.
  • Provide practical and emotional support to strengthen relationships, reduce risk, and encourage resilience.
  • Coordinate multi-agency interventions for families experiencing complex challenges.
  • Deliver joined-up, wraparound support, ensuring plans are collaborative and responsive to family needs.
  • Focused on early intervention, you will support parents and carers to address risks such as safeguarding, parenting challenges, antisocial behaviour, debt, and substance misuse.

About You

  • Passionate about delivering fantastic outcomes for people, you will have a background working within housing, social care, or family support.
  • Proven experience working directly with families or individuals experiencing multiple and complex needs.
  • Knowledgeable on safeguarding principles and multi-agency working to protect vulnerable adults and children.
  • The ability to manage caseloads effectively, prioritise work, and balance competing demands.
  • Excellent interpersonal skills to easily build rapport, manage conflict, and engage families positively.
  • Comfortable working with community partners and commissioned services to enhance local provision and capacity.
