Job Summary
Think Ahead Community Stroke Group are looking for an Outreach Worker to work on the BLAST initiative, (Building Lives After Stroke Team)
An Outreach Worker is required within BLAST, supporting stroke survivors. They will work within the community to help individuals rebuild independence, confidence, and quality of life following a stroke. The role involves providing practical, emotional, and social support, ensuring service users can access services, engage in their community, and continue their rehabilitation journey.
Key Responsibilities
- Provide one-to-one support to stroke survivors in their own homes and community settings.
- Assist individuals in achieving personal goals related to independence, rehabilitation, and social inclusion, including where relevant learning and training and employment.
- Support service users to access health services, community resources, and support groups.
- Encourage participation in social, recreational, and community activities to reduce isolation.
- Assist with communication challenges, including supporting individuals with aphasia within an advocacy role only.
- Reinforce rehabilitation strategies provided by healthcare professionals (e.g., exercises, routines).
- Provide emotional support and encouragement to build confidence and resilience.
- Monitor wellbeing and report any changes in condition or concerns.
- Maintain accurate case notes and reports.
- Work collaboratively with families, carers, and multidisciplinary teams.
- Promote safeguarding, dignity, and person-centred care at all times.
Working Conditions
- Primarily community-based, including home visits and local venues
- Will involve travel within the local area
- Flexible working hours, including occasional evenings or weekends
- Lone working
- Hybrid working
Initially a 12 month contract with the possibility of extension - funding dependant
