Little Haven is a home for 41 residents and is situated just a short stroll from the village of Dibden Purlieu, within easy reach of Hythe and its ferry crossing to Southampton. The home is split over three floors with lift access to each floor. The en-suite rooms are all light, airy and benefit from stunning views of the gardens, an acre of wood and terraced gardens. Everwood is a charity that exists solely to provide high-quality care for older people in our community. We offer nursing and residential care at Little Haven and at our sister home Heartland Manor we offer dementia and residential care. About You As a Care Practitioner you will be working alongside the Registered Nurse to ensure the smooth running of the shift by providing day to day management for the care team. You will take responsibility for allocations and break times and will work alongside the team to provide the support required to ensure the best quality of care is provided to our residents. Working with those involved in the residents care, its about developing, reviewing, and updating the residents personal care plan to meet their changing physical, social and psychological needs. You will work closely with the Registered Nurse to provide clinical interventions as required, as well as administering medication and carrying out risk assessments. About the Role You will assist the Nursing and Management Team to assess, plan, implement and monitor care in line with CQC guidelines and the homes guidelines Competently assessing residents on a daily basis using the National Early Warning Signs (NEWS) to recognise any deterioration in clinical conditions of a resident by monitoring Blood Pressure, Temperature, Height and weight, Pulse rate and rhythm, Blood glucose, fluid charts, obtaining urinalysis and specimens, taking blood specimens as required Undertaking clinical interventions which you have been deemed competent to undertake. Such as, Syringe Driver management, insulin administration, venepuncture and canulisation, stoma care, oral suctioning, tissue viability. Ensuring accurate record keeping at all times, ensuring compliance with CQC, relevant legalisation, and in line with relevant Wilverley policies and procedures Make contact with relevant health care professions to support the health and wellbeing of the resident, including supporting GP visits and accurately reporting back and recording outcomes. Competently administrating medication in line with the NICE guidance Ensure relevant handovers are carried out for all staff, taking into account different shift patterns, ensuring that staff are kept up to date with any changes in a residents condition Act as a complaints champion, ensuring the organisations complaints policy is adhered too Communicate effectively with staff, residents and their families and visiting professionals within the limits of competency and confidentiality Ensure all opportunities for individuals to be engaged in meaningful activities and allocating staff to support activities when required. Assist in the pre-admission assessment for perspective residents. Once a final decision has been agreed with the Home Manager/Deputy Manager you will then support a smooth transition into the home. Complete and review person centred care plans to involve residents and others involved in their care Actively promote and participate in the Resident of the day programme to ensure accurate up to date records are maintained. Taking responsibility for allocated service improvement activities Ensure the principles of the Equality and Diversity Act underpin yours and your teams behaviours at all times, actively challenging when this is not the case. Essential Core Skills – Level 3 Qualification – Ability to lead a team – Be proficient at using computer systems and electronic devices Desirable Core Skills – Previously held a post as a Senior Carer / Care Practitioner in a regulated setting – Previous experience of supervision and appraisals.
