Due to population growth, The Wilson Practice is excited to offer a new opportunity for an enthusiastic and compassionate Advanced Care Practitioner to join our friendly and progressive team. We welcome applications from experienced Advanced Nurse Practitioners (ANP / ACP) who hold a Master's degree (MSc) in Advanced Clinical Practice or equivalent, and Prescribing Paramedics with primary or urgent care experience.

Main duties of the job:
Your Role: You will be a key part of our on-the-day team, working autonomously but as part of a collaborative team to provide excellent, same-day patient care. The role includes telephone triage and face-to-face consultations for minor illness and acute needs.

Located in stunning Hampshire, we are a friendly, sociable, and supportive practice serving over 17,200 patients. We have 2 GP Partners, 14 Salaried GPs, 2 ANP, 2 ACPs, 5 Practice Nurses, and 3 HCAs, with excellent Reception, Admin and Management support. We maintain an open-door, team-focused culture and are a proud training practice for GP Registrars, FY2s and Nurse Trainees.
Job responsibilities:
- Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness.
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from a physiological and psychological perspective.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs.
- Support patients to adopt health promotion strategies that promote healthy lifestyles.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
- Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Recognise and work within own competence and professional code of conduct.
- Produce accurate, contemporaneous, and complete records of patient consultation.
- Deliver care according to NSF, NICE guidelines, and evidence-based care.
- Act as one of a number of clinical leaders in the delivery of same-day care.
- Support staff development to maximise potential.
- Manage and lead on the delivery of specifically identified services or projects.
Person Specification:
- Registered Nurse, ANP, APP Qualified Prescriber (training can be provided for the successful candidate).
- Experience of working in a primary or urgent care environment.
- Use of EMIS Web.
- Disclosure and Barring Service check required.
