Are you organised, proactive, and passionate about making a difference in social care? We are looking for a Referral Co-Ordinator to manage the journey of new referrals into our services. Acting as the first point of contact for local authorities, NHS partners, professionals, and families, you will ensure referrals are handled efficiently and that people receive the right support at the right time.
What you'll do:
- Manage and track all incoming referrals and enquiries.
- Assess referrals against service capacity and suitability.
- Coordinate assessments and support smooth transitions into services.
- Work closely with operational teams to identify vacancies and match suitable individuals.
- Maintain accurate records and reporting data.
- Analyse market demand and commissioning trends to support future service growth.
- Build strong relationships with stakeholders and provide regular updates.
What we're looking for:
- Experience in health and social care.
- Understanding of funding arrangements and commissioned services.
- Strong organisational, communication, and data analysis skills.
- Ability to manage multiple priorities and work collaboratively across teams.
- Confidence using digital systems, including Microsoft Office.
