Dartford MODEL PCN are seeking a PCN clinical pharmacist to join our existing team on a part time basis.
Main duties of the job include supporting in improving medicines management and medicines optimisation across the PCN. We are looking for someone with a passion in long term condition management, particularly in one of the following areas: respiratory or diabetes. Ideally, the post holder will hold a prescribing qualification although this is not essential.
