The successful candidate will be responsible for managing patients with long-term conditions as part of a large, experienced clinical team including GPs, ACPs, Practice Nurses, Pharmacists and Health Care Assistants. We will welcome part-time applicants to be considered for this opportunity.
We are a supportive training practice with 5 GP partners, Salaried GP, ACP and 2 GP Registrars. We have excellent supporting administrative and clinical teams including Practice Nurses, Healthcare Assistants and Pharmacy team. We have approximately 9,700 patients and operate from purpose-built premises. We are a member of the Tolson Care Partnership Network. We use TPP SystmOne and are high QOF achievers with a Good CQC rating.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations, sexual health services and well-person check-ups.
Main duties of the job:
- Large supportive in-house multi-disciplinary team
- The role will include proactive support for some or all of the areas:
- Diabetes and other Chronic Disease management
- Health Promotion and screening, Travel Health, Cervical cytology, Sexual Health, Baby, Childhood and Adult vaccinations
- Actively involved in the Primary Care Network
