Hours of Work: 18.75 hours per week - hours between Monday - Friday 09:00-17:00 with flexibility - specific work pattern to be discussed on successful appointment.

Pro Rata'd Salary: £14,887.84 per annum.

Project Description: Wrexham Floating Support service provides housing related support to individuals and families in their homes or community. You will be responsible for maintaining a caseload through responding to referrals, carrying out needs and safety assessments and support plans. You will support service users to achieve their goals by providing information, advice and guidance that enables service users to make informed decisions, signposting where needed, and coordinating planned service withdrawals.

Purpose of Role: To assist the Service Manager in the day-to-day running of the Services which fall under the responsibility of this role, ensuring objectives and targets are met in line with contract requirement and organisational values, working in partnership with internal stakeholders where required. To also assist in ensuring that staff and service users have the best possible support in place, allowing for the service to achieve its main aims and objectives. May also be required to carry own caseload.

This role is subject to an Enhanced DBS disclosure. Please note: Driving Licence and access to own vehicle is essential.
