Rydym yn dîm o weithwyr proffesiynol o wahanol sefydliadau sy’n dod i ddarparu Asesiadau, Ymyriadau a Chymorth mewn un lle. Rydym yn ei alw’n Abertawe Castell-nedd Port Talbot 360. Ein nod yw cynnwys unigolion sy’n agored i niwed mewn gwasanaethau cymorth a’u cyfeirio at gymorth perthnasol.
Pwrpas y Swydd: Bydd deiliad y swydd hon yn gyfrifol am ddatblygu rhaglen ymgysylltu â’r rhai sydd â phrofiad uniongyrchol o ddigartrefedd ledled Abertawe Castell-nedd Port Talbot fel rhan o brosiect Abertawe Castell-nedd Port Talbot 360. Bydd hefyd yn darparu cymorth sy’n gysylltiedig â thai i unigolion agored i niwed. Y nod yw hyrwyddo annibyniaeth, cynnal tenantiaeth a gwella llesiant.
- Datblygu a gweithredu cynlluniau cymorth sy’n canolbwyntio ar yr unigolyn ar y cyd â defnyddwyr gwasanaeth a chydweithwyr yn y Tîm Amlddisgyblaethol.
- Rhoi cymorth ymarferol gyda cheisiadau am dai, cytundebau tenantiaeth, cyllidebu a sgiliau byw bob dydd.
- Cysylltu â gweithwyr iechyd proffesiynol, gweithwyr cymdeithasol ac asiantaethau eraill i gydlynu cefnogaeth gyfannol.
- Cynhal asesiadau risg a chyfrannu at brosesau diogelu.
- Cefnogi cleientiaid drwy sefyllfaoedd argyfwng a thrawsnewidiadau, gan gynnwys rhyddhau o’r ysbyty neu lety symud ymlaen.
- Cynhal cofnodion cywir a chyfrannu at adolygiadau ac archwiliadau gwasanaeth.
- Ymgynghori â chleientiaid a’u cynnwys yn y gwaith o ddatblygu a darparu gwasanaethau.
- Defnyddio cydgynhyrchu i lywio’r gwaith o ddatblygu gwasanaethau ar bob cam ac ar bob lefel.
- Ymchwilio, datblygu, trefnu a darparu gweithgareddau ffurfiol ac anffurfiol i hwyluso cydgynhyrchu.
- Ymgorffori diwylliant o gyd-gynhyrchu a defnyddio’r egwyddorion i sicrhau bod unigolion â phrofiad byw yn cael cyfle cyfartal i fynegi eu barn ac yn ymwneud â dylunio a chyflawni pob agwedd ar y prosiect ac yn cael eu cynrychioli ar draws pob lefel.
