Come and join Greater Derby Primary Care Network (PCN), a large and forward-thinking PCN comprising 10 member practices serving a diverse population across Derby. We are seeking an experienced, proactive Clinical Pharmacist to deliver high quality, patient-centred clinical pharmacy services within a practice-based role. This role is not hybrid/remote; it is based within a practice within Greater Derby PCN practices in Derby City.

Reporting to HR Alexin and the PCN Management, the post holder will provide clinical pharmacy support across Greater Derby PCN in line with the priorities and direction set by the Greater Derby PCN Board. Main duties of the job

  • The successful candidate will work within their clinical competence to deliver structured medication reviews, support long term condition management, and contribute to quality improvement initiatives across the network.
  • This role offers scope for professional development, including independent prescribing (if not already qualified), and the opportunity to develop a specialist clinical interest aligned with PCN priorities.

About us

Greater Derby PCN consists of 10 GP practices with a collective list size of over 100,000 patients across the Derby area. The PCN has an established leadership team and currently employs Social Prescribers, Clinical Pharmacists, First Contact Physiotherapists, Health and Wellbeing Coaches and a centralised administration team. At present, Greater Derby PCN has chosen not to form a separate legal entity. All PCN staff are therefore employed by Alexin Healthcare CIC Limited.

The organisation is dedicated to delivering high quality health services to the community, improving patient experience and keeping Primary Care at the forefront of NHS reform.

Job responsibilities

  • Undertake clinical medication reviews for patients with single or multiple long term conditions requiring medicines optimisation.
  • Deliver Structured Medication Reviews (SMRs) in line with national and local guidance.
  • Work as part of the multidisciplinary team (MDT) within practices, participating in clinical meetings where appropriate.
  • Manage and review repeat prescription requests, resolving medication queries within scope of practice and ensuring appropriate monitoring is in place.
  • Implement prescribing changes in response to MHRA alerts, product withdrawals, shortages, and local/national guidance.
  • Maintain accurate and comprehensive patient records using appropriate clinical systems and coding.
  • Contribute to audit, service evaluation, and quality improvement initiatives, implementing recommendations where appropriate.
  • Provide patients with independent information and advice regarding medicines and support adherence in line with NICE guidance.
  • Monitor prescribing data and support practices to improve prescribing quality, safety, and cost effectiveness.
  • Contribute to the safe management of medicines following transfer of care from secondary care and care home settings.
  • Collaborate with the practice teams and PCN to support medicine optimisation goals and training of colleagues.
  • Support compliance with Care Quality Commission (CQC) standards for safe and effective care.
  • Support the practice in achieving the Quality and Outcomes Framework targets and Network DES IIF targets.

Person Specification

Qualifications

  • Completion of an undergraduate degree in Pharmacy.
  • Registration with the General Pharmaceutical Council (GPhC).

Experience

  • Minimum of 2 years experience as a pharmacist, evidenced within a professional portfolio.
  • Ability to follow legal, ethical, professional, and organisational policies and codes of conduct.
  • Experience of involving patients in decisions about prescribed medicines and supporting adherence in line with NICE guidance.
  • Previous experience working in a GP practice.
  • Completion of PCPEP course or in progress.
  • Independent Prescriber qualification or currently working towards.
