Main duties of the job:
- Face-to-face discussions with patients regarding medication issues as needed and appropriate.
- Carry out medication reviews and chronic disease medicines management.
- Prescribing medication following prescribing systems and policies.
- Liaison with community and hospital pharmacies.
- Medication reconciliation and medicines information according to guidelines.

Job responsibilities:
The Practice Pharmacist prescriber is an integral part of the clinical team, using and sharing pharmacy prescriber expertise to support the GPs and the whole practice in effective medicines management and optimal patient care. The role will be developed in close collaboration with the partners.

Experience:
- Have relevant clinical experience.
- Experience of working in a GP surgery.
Qualifications:
- Must be registered with the General Pharmaceutical Council (GPhC).
- Have relevant clinical experience and complete an accredited independent prescribing course.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
