You will deliver high-quality nursing care in a busy GP practice, supporting patients across chronic disease management, immunisations, screening, and preventative care.
We welcome candidates with COPD, Asthma, and Diabetes experience who are passionate about improving patient outcomes and promoting self-management of long-term conditions.
