The University of Nottingham Health Service is a General Practice based at Cripps Health Centre. The practice has a list size of over 40,000, making it one of the largest single-site NHS practices with a total staff number of approximately 100. In addition to the general practice, it runs a large and successful research team.
Main duties
- The post holder will work as part of a multi-disciplinary team in a partially patient facing role.
- Take responsibility for areas of chronic disease management, clinical medication reviews, and support general practice staff with prescription and medication queries.
- Provide support for the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care, and systems for safer prescribing.
- Provide expertise in clinical medicines advice while providing clinical leadership on medicines optimisation and quality improvement.
- Manage some aspects of the quality and outcomes framework and enhanced services.
Job responsibilities
- The post holder is a pharmacist who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice.
- They will be supported by a senior clinical pharmacist who will develop, manage, and mentor them.
- Provide primary support to general practice staff regarding prescription and medication queries.
- Help support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care.
- Ensure that the practice integrates with community and hospital pharmacy to improve patient outcomes and manage workload.
- The role is pivotal to improving the quality of care and operational efficiencies.
- The post holder will be supported to develop their role to become a non-medical prescriber if not currently qualified.
Person Specification
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
- Has previously completed the Primary Care Pharmacy Education Pathway (CPPE).
- Experience and awareness of common acute and long-term conditions likely to be seen in general practice.
- May hold or be working towards an independent prescribing qualification.
- Recognises priorities when problem-solving and identifies deviations from normal patterns.
- Able to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.
- Involves patients in decisions about prescribed medicines and supports adherence as per NICE guidelines.
Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
Salary: £46,000 to £51,000 a year, pay depending on skills and experience.
