We are looking for a dynamic, enthusiastic and driven person to join our nursing team! As a Practice Nurse, you will be an integral part of our multidisciplinary team, delivering patient-centred care across a wide range of clinical areas. You will assess, plan and deliver care to the highest standards, supporting patients with both acute and long-term conditions while promoting health and wellbeing.
We have a population of almost 19,000 patients with two sites and a varied demographic from deprivation to affluence. Our ethos is to provide exceptional patient care while also providing a supportive and collaborative environment for our staff.
About The Practice
- 2-site practice (St. Mellons and Roath) with GMS contract working on EMIS clinical system
- 7 Partners, 6 Salaried GP’s, 6 Nurses (including two prescribers), 1 HCA and 2 Prescribing Pharmacists
- Training practice supporting years 1, 3 & 5 medical students, nursing students, pharmacy students, independent prescribing placements and GPST rotation placements
- Integrated with allied and community teams
- Part of Cardiff East Cluster that collaborates to deliver projects to benefit our local community
Key Roles & Responsibilities of a Practice Nurse
- Treatment room and clinical duties, including but not exhaustive:
- Deliver high-quality nursing care in line with practice protocols and national guidelines
- Wound care, suture removal, venepuncture and management of minor injuries
- Administration of medications
- Immunisations and vaccinations, including childhood, flu, shingles and travel vaccines
- Perform ECGs and other diagnostic tests
- Cervical cytology and screening procedures
- Health checks including BP, BMI, and cardiovascular risk assessments
- Respond to medical emergencies including anaphylaxis and resuscitation
- Maintain safe storage, handling and stock control of vaccines and medications
- Support infection prevention and control standards
- Chaperone and assist clinicians where required
- Participate in occasional home visits where appropriate
- Asthma & COPD
- Diabetes and pre-diabetes
- Coronary Heart Disease
- Mental Health conditions
- Run and support disease-specific clinics
- Contribute to clinical targets and patient recall systems
- Smoking cessation, weight management, alcohol and substance misuse
- Cervical screening and child and adult immunisation programmes
- Cluster projects such as TeenTalk and Fit50
Professional & Administrative Duties, including but not exhaustive:
- Maintain accurate, contemporaneous clinical records using the practice clinical system
- Actively participate in practice meetings and clinical audits
- Engage in continuous professional development and annual appraisals
- Contribute to the development of practice policies, protocols and services
Requirements:
- Registered Nurse (NMC)
- Strong clinical skills across treatment room and chronic disease management (or willingness to train in CDM)
- Competence in vaccinations, including travel immunisations
- Experience performing ECGs and other investigative procedures (or willingness to train)
- Strong IT skills essential, including confident use of clinical systems
- Excellent communication and teamwork skills
- Commitment to high standards of patient care and confidentiality
- Ability to work across both sites with planned-in-advance rotas
- Experience in primary care or community nursing
- Experience in using clinical system EMIS
- Experience in chronic disease management and screening services
