- Manage a caseload of adults with a range of care and support needs, promoting independence, dignity and wellbeing.
- Complete Care Act assessments, reviews and reassessments, identifying individuals’ needs, strengths, risks and desired outcomes.
- Develop and review person-centred care and support plans, ensuring individuals have appropriate support in place.
- Work collaboratively with adults, families, carers and healthcare and other professionals to achieve positive outcomes.
- Assess and manage risk and safeguarding concerns, taking appropriate action to protect adults from abuse, neglect or exploitation.
- Support individuals to access community resources, housing, healthcare, benefits and other relevant services.
- Apply professional judgement when making decisions regarding eligibility, care packages, funding and support arrangements.
- Undertake Mental Capacity Act and best-interest work where appropriate, ensuring individuals’ rights and choices are respected.
- Maintain accurate and confidential case records, assessments, reports and documentation in line with statutory and organisational requirements.
- Participate in supervision, team meetings and multi-disciplinary working, contributing to continuous professional development and high-quality adult social care practice.
