Main Purpose of Job

The main purpose of this full-time contractual Developmental Disabilities (DD) Targeted Case Management (TCM) Coordinator of Community Services (CCS) position will be to provide a fixed point of entry into the service delivery system for individuals with developmental disabilities. This position will assist the individual in the preparation of the application and gathering of necessary documentation for DDA to make an eligibility determination. This position will also facilitate the development of the Person-Centered Plan (PCP) and monitor the PCP and its services. In addition, this position will complete a WPH assessment of the most integrated setting and identification of barriers and a plan to overcome the barriers in the State Residential Center. Per COMAR Regulation 10.09.48.05E a Coordinator of Community Services shall have: A Bachelor\’s degree from an accredited college or university in a Human Services field; OR An Associate\’s degree with two (2) years experience in a Human Services field; OR Seven (7) years experience in a Human Services field.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: One year of administrative staff or professional work in a health care setting. Notes: 1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. 2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

BENEFITS

Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits. Leave may be granted to a contractual employee at a rate of one hour for every 30 hours worked, not to exceed 40 hours per calendar year.
