This is a good opportunity to work for the community. We are looking for a professional and passionate pharmacist prescriber to join our friendly and supportive GP surgery.
Main duties of the job:
- Face-to-face discussions with patients regarding medication issues as needed and appropriate.
- Carry out medication reviews and chronic disease medicines management.
- Prescribing medication following prescribing systems and policies.
- Liaison with community and hospital pharmacies.
- Medication reconciliation and medicines information according to guidelines.

We are a large, friendly, dedicated practice with a determination to provide high quality clinical care for our patients, while also being highly committed to looking after our doctors. Due to retirement, an opportunity has arisen to join our friendly forward-thinking practice. We are situated in North East Wales on the Welsh/English border, with good connections to nearby cities, including the historical city of Chester, which is only 5 miles away. There are also excellent schooling options in the local area. We deliver our GMS contract over 3 sites: our main site is Hawarden Health Centre, with branch sites in Saltney and Buckley. Hawarden is located 6.7 miles from Chester and is 4 miles from the English border, close to motorway links providing direct access to Liverpool and Manchester.
Job responsibilities:
The Practice Pharmacist prescriber is an integral part of the clinical team, using and sharing pharmacy prescriber expertise to support the GPs and the whole practice in effective medicines management and optimal patient care. The role of Practice Pharmacist prescriber will be developed in close collaboration with the partners.

Experience:
- Have relevant clinical experience.
- Experience of working in a GP surgery.
Qualifications:
- Must be registered with the General Pharmaceutical Council (GPhC), have relevant clinical experience, and complete an accredited independent prescribing course.
