We are looking for a dynamic and flexible person to join our happy team in Soho, Central London.
Responsibilities
- Clinical practice: Assess, implement and evaluate treatment programmes that promote health and well-being, including individual treatment plans for patients requiring longer term treatments such as wound care, asthma, COPD, diabetes and hypertension, cardiovascular disease, mental health.
- Work with other healthcare professionals in monitoring and managing long term conditions, in line with national and local policies and practice needs.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice.
- Work with patients to support adherence to prescribed treatments; medication reviews.
- Provide information and advice on prescribed and/or over-the-counter medication, including medication regimens, possible side-effects and interactions.
- Support patients to adopt health promotion strategies that promote healthy living and apply principles of self-care, such as promoting exercise, improving diet and stopping risk-taking activities such as smoking and excess alcohol intake.
- Support and manage health needs of women presenting for family planning and cervical cytology consultations.
- Recognise, assess and possibly refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
- Meet the needs of patients presenting for opportunistic wound care.
- Other Practice Nurse duties as required by the Practice.
- Ensure clinical practice is safe and effective and remains within boundaries of competence, and acknowledge limitations.
- As a non-medical prescriber, follow the protocols set by the prescribing lead.

- Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary
- Chronic disease clinics (Diabetes, COPD, Asthma, Cardiovascular)
- New patient health checks
- Ensuring infection control measures are applied
