A new opportunity has arisen at Shrewsbury and Telford NHS Trust, to work with a friendly, compassionate and inspiring team as the department's Transitional Care and Outreach Lead. This role is key to the delivery of high quality and safe neonatal care.
The appointed member of staff will need to be a role model in their leadership and management within this area, being an approachable leader and an effective communicator. You will be responsible for leading and managing our outreach nursing team and our transitional care, working across professional boundaries and managing a variety of nurses, adults/children, and midwives. This role spans across community and secondary care services.
You will be required to build upon opportunities to further enhance family experience and to further integrate family integrated care, aiming where possible to keep family and baby together. The role will require the candidate to undertake regular visits and provide clinical care to maintain competence and skill set both in the hospital and in the community setting; therefore, car access and a full driving license is essential for this role.
