The Role
You will be working as a Social Worker principally to support young people in transition from Children's to Adult Learning Disability Services. Your role will involve supporting young people with learning disabilities and their families and carers to make a positive transfer to adult services. This would include liaising with Children's services and any other involved agencies. You would be involved in carrying out assessments, undertaking care reviews and supporting care planning. You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests. You would work as a Social Worker to protect and promote the interests of vulnerable people. Undertake safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best interest decisions for adults with a learning disability.
What we are looking for
- A qualified Social Worker with a qualification such as a BA, BSC Diploma or equivalent.
- Current registration with Social Work England.
- Experience of working in a health and social care environment.
- Good understanding of safeguarding protocols.
- Knowledge of relevant legislation.
- Committed to working co-operatively as part of a team.
