We have an exciting opportunity for an experienced Social Worker to join us! Become part of the Hospital Team, where you'll play a vital role in supporting residents through a safe and well-coordinated discharge process. Your efforts will help ensure that individuals transition smoothly from hospital care back into the community, with the right support in place to maintain their wellbeing and independence.
The Royal Borough of Windsor and Maidenhead (RBWM) is set in the wider county of Berkshire, one of the most economically productive and innovative areas of the country. Main duties of the job:
- Support & Advocacy: Help adults who may be vulnerable due to age, disability, illness, or mental health challenges to live independently and with dignity.
- Assessment & Planning: Assess individuals' needs and develop care plans tailored to improve wellbeing and safety.
- Safeguarding: Protect adults from abuse, neglect, or exploitation, working closely with other agencies when risks are identified.
- Coordination: Liaise with families, carers, health professionals, and community services to ensure holistic support.
- Empowerment: Promote choice and control, helping people understand their rights and access services.
What we are looking for:
- Relevant Qualification & Registration: Degree in Social Work and registration with Social Work England.
- Assessment & Care Planning Skills: Ability to carry out strengths-based assessments and develop person-centred care plans.
- Safeguarding Knowledge: Strong understanding of adult safeguarding procedures and legislation (e.g., Care Act 2014).
- Communication & Collaboration: Excellent interpersonal skills to work effectively with individuals, families, and multi-agency teams.
- Resilience & Professionalism: Capacity to manage complex cases, maintain professional boundaries, and adapt to changing demands.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
