RBKC Council is seeking a Social Worker (AMHP eligible) to provide compassionate, high quality support to older adults with complex mental health needs under Care Act duties. You will conduct home visits, lead safeguarding enquiries and coordinate with NHS colleagues to ensure safe, person-centred care.
Working in a hybrid office/remote setup, you will document interventions on Mosaic, participate in MDT meetings at St Charles, and benefit from supervision, training potential, and clear.
