We are seeking a motivated Practice Nurse to provide high-quality patient care within a busy practice. The role includes chronic disease management, immunisations, health promotion, wound care, cervical screening, and supporting long term condition reviews. The successful candidate will work collaboratively with the multidisciplinary team, demonstrate excellent clinical skills, and maintain high standards of patient care and confidentiality.

Main duties of the job

The main duty of a Practice Nurse is to deliver safe, effective and patient centred clinical care within a GP Practice, supporting the assessment, treatment, and ongoing management of patients' health needs. This includes managing long-term conditions, carrying out clinical procedures, promoting health and wellbeing, and working closely with the wider primary care team to ensure high-quality, coordinated care.

Job responsibilities

  • Management of long term conditions such as Asthma, COPD, Diabetes and hypertension.
  • Delivering immunisations and vaccinations (including childhood and travel where appropriate).
  • Cervical screening and women's services.
  • Wound care, dressings and removal of sutures.
  • Health Promotion, lifestyle advice and preventative care.
  • Infection prevention and control, including cold chain management.
  • Supporting QOF, audits, and clinical governance activities.
  • Work collaboratively with GPs, HCAs and wider multidisciplinary team.
  • Maintaining accurate clinical records in line with professional standards.

Person Specification

  • Registered Nurse (NMC)
  • Previous experience working as a Practice Nurse in General Practice
  • Chronic Disease management training and experience
  • Ability to work independently and manage own workload
  • Excellent communication and interpersonal skills
  • Up to date clinical knowledge and competencies.
  • Experience using SystemOne
