We are looking for two part-time CareNavigators/Receptionists to join our busy and friendly team, providing ahigh quality service to patients, doctors and other members of the PrimaryHealthcare Team. Working 20 hours per week across our sites at Hessle, Brough and South Cave, Bubwith and Holme on Spalding Moor (HOSM).

  • 1 x Care Navigator at 20hrs per week - mainly at HOSM & Bubwith, must be willing to work at any of our sites.
  • 1 x Care Navigator at 20hrs per week - mainly at Hessle with cover for Brough & South Cave, must be willing to work at any of our sites.

Candidates should be experiencedin delivering excellent reception support, maintaining professionalism anddiscretion at all times and be able to work independently and as part of ateam. Good communication skills and attention to detail are essential alongwith the flexibility to cover for annual/sick leave. Knowledge of SystmOnewould be an advantage.

You will have experience of working with thepublic, have a cheerful and polite manner, be able to multi-task and be a goodteam player. You must be able to contribute to the maintenance of patientconfidentiality at all times, inside and outside the Practice.

Main duties of the job

You will need to be a good communicator withexcellent interpersonal and customer service skills; especially under pressure.You will have a range of patient facing and administrative tasks, so excellentorganisational skills and time management are critical. A caring, empathetic characteris essential.

Your duties will include answering telephone calls,registering patients and general admin within the Practice. It is veryimportant to have a 'can do' attitude. It is also important to be flexible and able cover shifts when required.

Job responsibilities

Greet patients andvisitors to the Practice.

Book in patientsand visitors in line with Practice appointments and visitors procedures.

Respond and/orredirect all patient and visitor requests accordingly and ensure unresolvedand urgent matters are passed onto incoming colleagues at end of shift.

Opening incomingmail and distribute as per protocol.

Booking, amending,and cancelling patient appointments.

Recording patientinformation onto the clinical system.

Ensure waiting roomtemperature is comfortable for patients and responsible for tidying thewaiting area (magazines, toys etc.) after each surgery

Answering incomingtelephone calls, ensuring calls are documented and redirected accordingly.

Care Navigate allpatients, including referring patients to outside providers.

Book appointmentsusing Improving Access System.

Must be flexible toprovide holiday and sickness cover on a regular basis.

Maintenance of bothcomputerised and manual filing systems (scanning, filing etc.).

Ring Tasks sent byClinicians using the flagged system.

PrescriptionsHand paper copies to patients when Dispensary is closed.

Make tea/coffee forcolleagues, doctors and visitors as required

Work safely at alltimes in accordance with Legislative requirements and Practice Policy andProcedures.

AdministrationDuties

Registrations, NewPatient Questionnaires and Deductions must be kept up to date.

Any otheradditional administration duties as required.
